Naima prepares to give a shot of insulin to her grandmother. Her youngest uncle, Teddy, and his girlfriend are there with her, but Naima is uncomfortable around Teddy and insists on doing the injection herself, as those two spend much of their time doing drugs. Teddy watches her as she administers the shot. He makes her nervous by saying that she could do it wrong and send her grandmother into a coma, even though she has been giving the injection twice a day for a year. Teddy lives with his grandmother and shows no sign of leaving; he wants the needles for his drug habit. Naima is twelve and does not believe herself to be particularly smart, but this is one task she knows she can perform perfectly.
Naima visits her cousin Tony, who is home from a prestigious boarding school. He lives with Teddy and his grandmother. He refuses to help Naima clean the kitchen. Naima is frustrated because her mother raised him and the entire family supported him as he took his tests and completed his interviews. She speaks to her mother about it and her mother insists that Tony is special and has a chance of making it out of poverty and living in real happiness. This hurts Naima because she believes that the entire family is already happy.
The next day, as Naima is giving her grandmother her insulin shot, Teddy barges into the room and insists that he is sick. He crawls into bed with Naomi's grandmother and asks her for money. She asks if he needs money for medicine and he lies by saying yes, even though it is clear that he wants it for drugs. At the same time, he steals the syringe without Naima noticing.
